Abstract Malignant phyllodes tumors of the breast are fairly rare and fast-growing tumors. They not sensitive to chemotherapy or hormonal therapy. Therefore, primary treatment for malignant is wide surgical excision. Herein, we report a case study which featured 26-year-old woman presented with giant tumor measuring 20 × 17 13 cm. In order reduce chance local recurrence, these types usually involves extensive excision at least 1 cm margins. The patient underwent mastectomy negative margins, left large skin defect 25 15 Repair such challenge surgeons. This first reported in “kiss” flap was successfully used repair created after resection . kiss could be considered as an effective simple method chest wall defects
